Antibiotic prophylaxis for prosthetic joints and orthopedic implants

For patients with prosthetic joint implants, prophylactic antibiotics are not recommended before dental procedures to prevent prosthetic joint infection. Total joint infections can result, leading to poorer conditions of the mouth. Patients are immunocompromised two years following the joint replacement due to inflammatory arthropathies such as arthritis.
We also advise patients with comorbidities, such as previous prosthetic joint infection, malnourishment, hemophilia, HIV infection, insulin-dependent diabetes, and malignancy. For patients with a history of complications associated with their joint replacement surgery undergoing dental procedures that include gingival manipulation or mucosal incision, prophylactic antibiotics should only be considered after consultation with the patient and orthopedic surgeon. To access the patient's medical status, a complete health history is always recommended when deciding on antibiotic prophylaxis. We also focus on individual patient's circumstances, and preferences are considered when deciding whether to prescribe prophylactic antibiotics before dental procedures.
Suggested antibiotics
Cefazolin is administered at 1 dram and ampicillin 2 grams, either intramuscularly or intravenously 1 hour before the dental procedure. For patients not allergic to penicillin, cephalexin, or amoxicillin, 2 grams orally is advised 1 hour before the dental procedure. Patients are not allergic to penicillin and unable to take oral medication cefazolin or ampicillin are advised.
For patients who are also allergic to penicillin, we have you sorted. Clindamycin is administered at 600 mill grams orally one hour before the dental procedure. Patients who are allergic to penicillin and unable to take oral medications are given clindamycin administered at 600 mg one hour before the dental procedure. Antibiotic prophylaxis can be recommended in dental procedures involving, manipulation of the gingival tissue, manipulation of the periapical region of the teeth, and during perforation of the oral mucosa.
